COATS - On Hold Form for AOD Service Providers

Welcome to the COATS On Hold Request Form for AOD treatment agencies. Please complete this form when your consortium or agency is unable to provide treatment appointments within 10 business days of receiving the referral from COATS. This will assist COATS in finding alternative treatment options for clients waiting for appointments.

Please note, the KPI for forensic clients is five business days from receipt of referral from COATS; and two business days from date of release for pre-referred prisoners. 

To assist in managing high demand in forensic referrals at a local level, it is important that agencies advise their Department of Health and Human Services (DHHS) representatives at a divisional level of any capacity issues, and consult with their consortia lead and partner agencies; prior to requesting that referrals are put on hold for a particular location or period of time. 

Please be aware that catchment-based Intake and Assessment providers must not place assessments on hold. Not all counselling services need to be placed on hold if an agency is able to provide complex only, or standard only, for a period of time. This also applies to non-residential withdrawal complex and standard.
