COATS Prison Referral Form - Assessment and Transition Coordinators

This referral form is to be used by Assessment and Transition Coordinators (ATCs) only.

If you are a Parole Officer and wish to refer a client who either has been granted parole or is awaiting their parole application outcome, please use the COATS Parole Referral Form - Parole Officer.

If you are from a ReConnect program (ACSO, JSS, VALS or VACRO) please use the COATS StepOut Referral Form - ReConnect.


This referral form is to be submitted by the Assessment and Transition Coordinator (ATC) once a client's release details, including release date, have been confirmed. This referral can be used to obtain a drug and alcohol assessment appointment for clients sentenced to a Parole Interstate or those on straight release from prison. 

StepOut: Clients on straight release from prison are eligible to participate in the Stepout program, which provides the opportunity for clients to voluntarily engage in assessment and/or treatment to address drug and alcohol concerns. Please note COATS may not accept StepOut referrals in the following instances:

  • If referrals are not received within 10 business days of a release date. 
  • If referrals are received greater than six months prior to the release date. 

Parole Interstate: If referrals are not received within 10 business days of a release date, COATS will notify the relevant CCS location, requesting they contacts COATS once the client is released to arrange an assessment in the community.

COATS will endevour to assess all clients in custody if referred within reasonable time frames. If an interpreter is required, COATS will arrange an assessment at the COATS office nearest to the client after their release from custody. 

Once you have submitted this referral form, COATS will review the details in order to arrange an assessment at the custodial facility for the client.
